CYSA-N Registration – Fall 2009

 

Required paperwork for CYSA-N registration is largely the same as in previous seasons. 

 

For each Team:

  1. Team fees, payable to MVLA SC, are set based on a team’s priority in practice field allocation.

Ø  $250/player for highest priority practice field allocation.

Ø  $205/player for standard priority practice field allocation.

Ø  $170/player for lowest priority practice field allocation.

 

Notes:

Ø  All teams playing in the GSSL or NorCal playing leagues may deduct $250 from their total fees owed.  The per-player fees therefore drop by $250 / #players rostered.

Ø  All U17 and U18 teams playing Abronzino must add $350 to their total fees owed.   Games for these age groups are played in Morgan Hill, with referees supplied by the Abronzino league.  The per-player fees therefore increase by $350 / #players rostered.

Ø  Teams playing 8v8 pay fees that are one level lower than their priority.  For example, a team playing 8v8 who requests highest priority practice field allocation, will pay $205/player.

Ø  Fees may be submitted as a single team check or as individual checks from each player.

2.    Enclose all forms (arranged alphabetically by player last name) in a team envelope with the registration sticker filled out in its entirety.

 

For each player:

 Paper clip together:

1.    Two (2) copies of the 1601 Form, revision 12/08, with at least one wet ink signature in non-black ink.  Use the player’s full name; no nicknames.  Include the month and day of the mother’s birth date.  This is needed to generate a player ID.

2.    One (1) clear copy of the birth certificate or passport for all “new to CYSA” players (i.e. players not on your fall 2008 or spring 2009 rosters.)

a.    One (1) completed copy of the 1627 Form (Foreign Document Translation Form), revision 5/2005 if the proof of age document is not in English.

3.    One (1) 1”x1.5” photo (no hats, sunglasses, polaroids, laminated photos or photos from old player passes) labeled with the name on the back.

 

Forms should be arranged in alphabetical order by last name.

 

For each Staff:

 

Each staff member should complete the online registration and risk management disclosure.  Clicking the "Submit" button will generate a printable 1628 form that must be updated with the staff member's activity (coach, assistant coach, etc.) and a wet ink signature.

Paper clip together:

1.    Two (2) copies of the 1628 Form, generated from the online registration step, with at least one wet ink signature.

2.    One (1) clear copy of the most recent coaching license for all adults that have obtained a higher level than is currently in the CYSA database.  Your team envelope has a report listing the currently recorded license level for all MVLA adults.

3.    One (1) 1”x1.5” photo (no hats, sunglasses, polaroids, laminated photos or photos from old player passes) labeled with the name on the back.
