US Club Registration for 2010-11

 

US Club registration is completely separate from CYSA.  US Club registration is required for participation in NorCal Premier events such the NorCal State Cup, NorCal Fall League, and NorCal Spring League.  Many tournaments also accept US Club credentials.

New US Club Teams
If your team is new to US Club, please email the following information to the MVLA club registrar, who will then create your team record and assign staff login credentials.

Each team is limited to one individual per role.  Only staff members assigned to one of these roles will appear on the official team roster and have access to the team via the US Club website.  You may register additional staff, but please verify with appropriate tournament or league officials to determine the privileges of unrostered staff as this will vary for each event.

Adding Players to a Team Roster

Any rostered staff can login to the US Club website and add players to the team roster by using the Player Management tab. 

·         Upload a valid proof of age document for each player.  You need not upload a proof of age document for players who have continuously held a player pass with Foothill FC.

·         Optionally upload a photo for each player.  If photos are uploaded, US Club will generate passcards with the photos.  You will not have to glue photos to passcards after they are returned by US Club.

 

Staff Risk Management

Each staff member must complete the online risk management form.  When completing this form:

1.    The Type of Membership is Club, Academy or Recreational Club.

2.    The Club is Foothill FC, #1890



Requesting Passcards

Once all players have been added to the roster and all staff members have completed the online risk management form, send an email to the MVLA club registrar to request passcards.  The cost of registration is $15/staff, $16/player (U12+), and $12/player (U11 and below).  Prior to emailing your request:

  1. Ensure that all staff members have completed the online risk management form
  2. Ensure that the POB Attachment for all players is either Validated or Attached.
  3. Where applicable, ensure that players have been released by their previous club.  It is not sufficient to release players from a team roster only.

Effective immediately, both the online risk management form and the staff passcards are good for two years.  Player passcards continue to be valid for one year only.
